چکیده

Abstract Infective endocarditis is a relatively rare disease with an estimated mean incidence of 3.6–7.0 cases per 100,000 subject–years. Aortic bicuspid disease, the most frequent congenital anomaly, one main risk factors. Heart failure, uncontrolled infection (fistulas, abscesses, persistently positive blood cultures) and systemic embolization are among feared complications endocarditis: these constitute indication for emergency/urgent cardiac surgery. An example extremely demolitive surgery Commando or "UFO" procedure, which consists in double replacement aortic valve mitral reconstruction mitro–aortic junction. The clinical case concerns 36–year–old man who went to emergency room high fever shivering. Having been found cultures MSSA having emerged vegetation by echocardiogram, diagnosis infective was made. During hospitalization, multi–organ cardioembolic phenomena (e.g. splenic, cerebral) hemorrhagic cerebral infarction occurred: therefore, accordance current guidelines, conservative treatment temporarily opted for. However, following days deterioration clinical–haemodynamic picture such as require patient thus subjected UFO procedure mitral–aortic Severe bi–ventricular dysfunction first time at post–operative echocardiographic control, need mechanical support circulation (AV ECMO, Impella). remainder hospitalization characterized progressive recovery ventricular function (LVEF 16% 44%), therefore discharged discrete hemodynamic compensation, despite multiorgan sequelae (hepatic, neurological, renal).

A rare case of prosthetic aortic valve endocarditis complicated with multiple fistula to peri aortic structure

Multiple aorto-cardiac cavity communications is very rare but important complication of prosthetic aortic valve endocarditis. The case below illustrates multiple aorto-cardiac cavity fistula formation following prostethic aortic valve endocarditis presented with slowly progressive symptoms of heart failure. A brief review of surgical reconstruction and the existing literature are presented, i...

Destructive aortic valve endocarditis from Brucella abortus: survival with emergency aortic valve replacement.

Brucella abortus infection of the aortic valve caused acute aortic regurgitation leading to severe left ventricular failure in a 62-year-old man. He made an excellent recovery after emergency aortic valve replacement. This is the third reported case of successful heart valve replacement for Brucella endocarditis and the second such case involving the aortic valve.

Successful moxifloxacin prophylaxis against experimental streptococcal aortic valve endocarditis.

OBJECTIVES Studies related to the prophylactic efficacy of fluoroquinolones against infective endocarditis are scarce. The aim of this study was to evaluate the efficacy of moxifloxacin, a quinolone active in vitro against Gram-positive cocci, in preventing streptococcal aortic valve endocarditis. METHODS Non-bacterial thrombotic endocarditis of the aortic valve was induced by the insertion o...

Successful trovafloxacin prophylaxis against experimental streptococcal aortic valve endocarditis.

Single-dose trovafloxacin (15 mg/kg given intravenously [i.v.]) and ampicillin (40 mg/kg given i.v.) protected 38 and 33% of animals challenged with an ampicillin-tolerant strain of Streptococcus oralis, respectively. As a double-dose regimen, trovafloxacin afforded total protection (100%; P < 0.001 versus controls). Trovafloxacin is the first fluoroquinolone effective in preventing experimenta...
